Thus, the summative assessment for this module consists of:


  • A 2000 word assignment:



Discuss the impact of living with one long term condition on the person, their families/carers. Explore the care and support required and how health care professionals can promote self-management of their condition.

  • This module aims to develop the knowledge, skills and understanding of the management of long term condtionsand the impact on the person, their family and how to activetly support and empower self care
