  • A 2000 word assignment:



 

“Discuss the impact of living with one long term condition on the person, their families/carers. Explore the care and support required and how health care professionals can promote self-management of their condition”

 

Formative assessment

 

Students will submit a draft plan of their assignment with a paragraph including references for tutor feedback

  • This module aims to develop the knowledge, skills and understanding of the management of long term conditions and the impact on the person, their family and how to actively support and empower self care
